1. Provider agrees to provide a safe and loving environment for all children in her care.
2. Parents are required to provide school snacks.
3. Provider is not responsible for delivering/retrieving children from alternate care programs.
4. Parent's must give permission in writing to the Provider of any alternate person picking up child. Please note: they
will be asked for picture I.D.
5. Medication will only be administered by perscription and only when proper forms are completed.
6. All medications will be stored out of reach of the children and must be handed directly to the Provider by the Parent.
7. It is the Parent's responsibility to notify the Provider of any known allergies in case of accidental exposure.
8. Children cannot attend daycare if they have a fever of 101 deg F, diarrhea, vomiting or a contagious disease i.e: chicken
pox, measels, pink-eye or any unexplained rashes not cleared by a physician.
9. PLEASE NOTE: 2 weeks notice of termination by either party is required or deposit is forfeited.
